A Brazilian butt lift (BBL) is a procedure in which the surgeon removes your own fat with liposuction and transfers it to the buttocks. Despite the name, no skin is lifted and there are no long incisions like in a traditional lift. The “lift” comes from added volume and a more defined curve between the waist and the buttocks.

The BBL is hugely popular and also widely debated. In this guide we walk through the procedure step by step, the real risks and what safety guidance says, recovery week by week, how to sit afterwards and what to plan if you are travelling to Yerevan for surgery.

Brazilian butt lift procedure: what happens in surgery

A BBL has three stages, all done in one operation:

  1. Liposuction. Fat is removed from donor areas, most often the abdomen, flanks, lower back, upper back or thighs. This alone refines your contour by narrowing the waist.
  2. Fat processing. The harvested tissue is cleaned of fluid and damaged cells so that only viable fat is transferred.
  3. Fat grafting. The fat is injected through small openings with thin cannulas and distributed to create the planned shape.

The operation is usually done under general anaesthesia and typically takes 2–4 hours, depending on how many areas are treated. Many patients stay one night in the clinic, but your surgeon makes that call.

Who is a good candidate for a BBL?

Good candidates usually:

  • have enough fat in donor areas;
  • are at a stable weight and not planning major weight loss;
  • do not smoke, or will stop for the weeks before and after surgery;
  • have no significant bleeding disorders or serious uncontrolled health conditions;
  • accept that some fat will not survive and that the result develops over months.

In very slim patients a BBL may not produce a visible change. In that case the surgeon may discuss implants instead.

Brazilian butt lift risks: what you need to know

Any honest conversation about the BBL has to start with fat embolism. When fat is injected deep into the gluteal muscle, the cannula can injure large veins, and fat can travel through the bloodstream to the lungs. This is rare but can be fatal, and it is the reason the BBL was once described as one of the riskiest cosmetic operations.

After reviewing these cases, the Aesthetic Surgery Education and Research Foundation (ASERF) and other plastic surgery societies issued guidance: inject fat only into the subcutaneous layer, above the muscle, keep the cannula angled correctly and never go beneath the gluteal fascia. Many surgeons also use real-time ultrasound to confirm cannula position. Published data suggest that the move to subcutaneous-only technique has reduced serious complications.

Other possible risks include:

  • uneven fat survival, asymmetry or contour irregularities;
  • seroma (fluid collection) in liposuction areas;
  • infection and fat necrosis (firm lumps where fat cells have died);
  • deep vein thrombosis;
  • changes in skin sensation and pigmentation at entry points.

Patients often ask whether a BBL or butt implants is better. They are different tools rather than rivals. A BBL reshapes your whole silhouette, because the waist and flanks get slimmer as the buttocks get fuller, and the result feels natural. But it needs donor fat, and some volume is lost in the first months.

Implants give predictable volume even in very slim patients, but they are a foreign body with their own risks, such as implant shift, infection and wound problems in the natal cleft. Sometimes surgeons combine both approaches. Which option suits you can only be decided at a consultation after an examination.

Everyone heals differently, but the overall pattern usually looks like this:

Donor areas often feel like a very intense post-workout ache, while the buttocks are sensitive to pressure. Your doctor prescribes pain relief. Compression garments are usually worn for 4–6 weeks, sometimes longer.

How to sit after a BBL

Transferred fat is fragile at first: it needs new blood vessels to grow into it. Pressure from sitting can cut off that supply and cause fat to die. So:

  • direct sitting on the buttocks is usually avoided for 2–3 weeks, and some surgeons recommend longer;
  • when you must sit, for example in the car or bathroom, a BBL pillow shifts the weight onto the backs of your thighs;
  • sleep on your front or side;
  • in a car, a reclined back seat with a cushion under your thighs is the most comfortable option.

A practical tip: plan your workspace before you travel. A laptop on a high chest of drawers or a kitchen counter makes the first weeks much easier.

Massage after a BBL

Many clinics recommend lymphatic drainage massage after liposuction to help reduce swelling and firmness in donor areas. The buttocks are a different story: deep massage of the grafted area is usually avoided in the early weeks so as not to disturb the fat while it settles.

When to start, which type of massage and on which areas is up to your surgeon. If you are flying home, ask for written instructions you can show a local lymphatic drainage therapist.

How to prepare for a Brazilian butt lift

  • Complete the blood tests and checks your clinic requests, including clotting tests.
  • Stop smoking and nicotine at least 4 weeks before and after surgery, as nicotine impairs healing and fat survival.
  • Review every medication and supplement with your doctor, since some thin the blood.
  • Keep your weight stable in the months before surgery.
  • Buy compression garments in the right size and a BBL pillow in advance.

BBL in Yerevan: how long to stay and when to fly

The flight home needs planning: you will be sitting, which puts pressure on the buttocks and raises the risk of blood clots.
