Round vs teardrop implants, silicone vs saline: the key differences
Implant shape, fill and surface explained in plain language, with honest information on BIA-ALCL and how surgeons choose.
Round vs teardrop implants, silicone vs saline, smooth vs textured: these are the three technical choices that cause the most confusion before breast augmentation. None of them is “best” for everyone. Each option has its own strengths, limits and risks, and your surgeon makes the final recommendation based on your anatomy and the result you want. Below we compare them plainly, including what you should know about BIA-ALCL, and explain how it works if you have surgery in Yerevan, Armenia.
Round vs teardrop implants: what is the difference?
Round implants look the same from every angle. When you stand up, the filling shifts slightly downwards, so the result can still look natural, with a little more fullness in the upper part of the breast. If a round implant rotates inside its pocket, nothing changes visually.
Teardrop (anatomical) implants are slimmer at the top and fuller at the bottom, made with a firm, form-stable gel. They can help in patients with very little tissue or in some reconstruction cases. The downside: if a teardrop implant rotates, the breast shape can look distorted. To reduce that risk they are usually textured, which brings its own considerations (see below).
| Feature | Round | Teardrop (anatomical) |
|---|---|---|
| Shape | Symmetrical | Sloped, fuller at the bottom |
| If it rotates | No visible change | Can distort the breast |
| Surface | Smooth or textured | Almost always textured |
| Upper pole | Slightly fuller | Flatter, more gradual |
| How often used | Very widely | More selected cases |
In round vs anatomical implant comparisons, many patients and even surgeons find the difference subtle once size and placement are right. That is why volume and profile deserve your attention first.
Silicone vs saline implants: which feels more natural?
Silicone implants are filled with a cohesive gel and usually feel closer to natural breast tissue. They are the most widely used type in Europe and many other regions. If the shell ruptures, the gel tends to stay inside the scar capsule, so the rupture can go unnoticed (a “silent rupture”). For that reason, regulators such as the US FDA recommend periodic ultrasound or MRI screening some years after surgery.
Saline implants are filled with sterile salt water during surgery, sometimes through a smaller incision. If one ruptures, it deflates and you notice quickly; the saline is absorbed harmlessly. On the other hand, they tend to feel firmer and ripple more, especially in slim women. Availability varies from clinic to clinic.
- Feel: silicone usually feels more natural.
- Rupture detection: saline is obvious; silicone needs imaging.
- Rippling: more common with saline.
- Incision: saline may allow a slightly shorter scar.
Smooth vs textured implants
Implant shells can be smooth, micro-textured or macro-textured. Texturing was introduced to help the implant grip the surrounding tissue and stay in position, which matters most for teardrop shapes.
Smooth implants move more freely within the pocket and are increasingly used with round shapes. Textured implants, especially heavily textured ones, have been linked to a higher risk of BIA-ALCL. Many surgeons now prefer smooth or finely textured surfaces, although the choice still depends on the individual case and the manufacturer.
What is BIA-ALCL and how worried should you be?
BIA-ALCL (breast implant-associated anaplastic large cell lymphoma) is a rare lymphoma of the immune system, not breast cancer. It has been linked mainly to textured implants and only very rarely, if at all, to smooth ones. Key facts:
- It typically appears years after surgery, most often as persistent swelling caused by fluid around the implant.
- When caught early, treatment usually involves removing the implant and the surrounding capsule.
- Health authorities do not recommend removing implants in women without symptoms purely because of this risk.
- In 2019 one major manufacturer withdrew its macro-textured implants from the market.
If you notice swelling, a lump or any change in an augmented breast, see a doctor, even many years later.
How your surgeon decides
The recommendation combines your measurements, tissue thickness, placement (above or below the muscle), the look you want and the surgeon’s own experience with each type. Useful questions to ask:
- Which shape, fill and surface do you recommend for me, and why?
- Which manufacturer does the clinic use, and what warranty comes with it?
- Will I receive an implant card with the model and serial numbers?
- What imaging do you recommend in the years ahead?

Having breast augmentation in Yerevan
Implant brands available in Yerevan depend on the clinic. Medarmenia checks which manufacturer will be used and makes sure you leave with full implant documentation. Keep it safe: any doctor who sees you at home will want it.
Most patients stay around 5 to 7 days, and the surgeon decides when you can fly after your first check-up. Back home, follow the instructions on your support bra, avoid heavy lifting for the period your surgeon advises and get in touch quickly if anything changes.
Frequently asked questions
Do teardrop implants look more natural than round ones?
Not necessarily. With the right size and placement, round implants can look very natural too, and many people cannot tell the difference in everyday clothing.
How long do silicone implants last?
They do not have a fixed expiry date, but they are not lifetime devices. Many women need a replacement or removal at some point, so regular check-ups matter.
Are textured implants dangerous?
They carry a higher risk of BIA-ALCL than smooth implants, though the condition remains rare. If you have them and no symptoms, removal is not routinely advised, but follow-up is.
Can you feel a saline implant?
Saline implants often feel firmer and ripple more than silicone, particularly if you have little natural tissue covering them.
Can I choose my implant brand?
You can ask and state a preference, but availability depends on the clinic. We confirm the options with you before you book your trip.
