Buccal fat removal: who it suits and the long-term risks

Buccal fat removal can slim the cheeks, but it is permanent and not for everyone. Who it suits, what recovery is like and how it ages.

Buccal fat removal takes out part of the buccal fat pads, deposits of fat in the cheeks, to slim the mid-face. It is done through the inside of the mouth, leaves no visible scar and is quick. But it is permanent, and it does not suit everyone: in a face that is already slim, it can lead to a hollow, gaunt look as you age. Here we explain what it involves, who is a good candidate and what to ask if you are considering it in Yerevan, Armenia.

What is buccal fat removal and how is it done?

Buccal fat pads are encapsulated cushions of fat between the cheek muscles. Their size varies a lot between people and barely changes with diet, which is why some slim people still have round faces.

The procedure is usually done under local anaesthesia, sometimes with sedation, and takes 30 to 60 minutes. The surgeon makes a small incision of about one centimetre inside the cheek, near the upper molars, gently teases out the portion of fat that comes forward and closes with dissolvable stitches. Many surgeons now prefer partial removal, a more conservative approach, rather than taking as much as possible.

Who is a good candidate?

It may make sense if:

  • your face is round and full despite a normal, stable weight;
  • the fullness is clearly in the cheek, not the jaw or the masseter muscle;
  • you are an adult whose face has finished maturing (many surgeons prefer to wait until your mid-twenties);
  • your expectations are realistic: the change is subtle, not a new face.

It is usually not recommended if your face is already slim, you have little facial fat, you are older and starting to lose volume, or the width comes from your jaw. In that last case, other options may be considered, such as treating the masseter with botulinum toxin or contouring with fillers.

Does buccal fat removal age you? Risks and consequences

This is the big question, and it shows up constantly in searches. Faces naturally lose fat with age and cheeks tend to hollow. Remove buccal fat from a face that had no excess and, 10 or 20 years later, the result can look tired or skeletal. That is why patient selection matters so much and partial removal has become standard.

Other risks to know about:

  • haematoma, infection or prolonged swelling;
  • asymmetry between the two cheeks;
  • injury to the parotid (salivary) duct, which opens nearby;
  • injury to branches of the facial nerve, rare and usually temporary;
  • over-hollowing, which fat grafting can only partly correct.

Removed fat does not grow back, so the decision is final. Your surgeon decides whether you are a candidate and how much to remove.

Buccal fat removal recovery day by day

For the first few days, avoid hard, spicy or very hot food, do not smoke and sleep with your head slightly raised. Strenuous exercise usually waits about 2 weeks.

Buccal fat removal, fat transfer or facelift: not the same thing

These procedures are easy to confuse. Buccal fat removal reduces volume in the lower cheek; facial fat transfer adds volume using your own fat; a facelift repositions sagging tissue. What to ask your surgeon before buccal fat removal

A thorough consultation is the best protection against a disappointing result. Bring these questions with you:

  • Is my round face really caused by buccal fat, or by my jaw, masseter muscle or general facial fat?
  • How much do you plan to remove, and why partial rather than full removal?
  • How do you expect my face to look in 10 to 15 years?
  • What would you do if I ended up with asymmetry or too much hollowing?
  • Would another approach, such as jawline contouring or simply waiting, suit me better?

If a surgeon advises against the procedure, take it as a sign of honesty rather than an obstacle. Good candidates are fewer than social media suggests, and a surgeon who turns down unsuitable patients is usually one you can trust.

Buccal fat removal in Yerevan: stay and follow-up

Is buccal fat removal different for men?

The technique is the same. With men, surgeons take particular care not to over-slim, so the face keeps a healthy, masculine look.

When will I see the final result?

Around 3 to 6 months, once swelling has settled. Before that, your face may look the same or even fuller.

Can it be reversed?

Not fully. If the cheek ends up too hollow, fat grafting can restore some volume, but it does not recreate the original situation.
